Patient's Rights and Responsibilities

By taking an active role in your health care, you can help us meet your needs as a patient. We ask that you (and your family) accept certain responsibilities so we can meet our shared goals.

Taking an active role in your own care means:

taking sensible precautions to remain healthy,
trusting the advice and information from your health care providers,
asking questions to clarify information, and
telling us what's important to you.

We ask that you:

Tell us about concerns you have about the safety and/or quality of your care.
Please inform reception of any changes to your address, home, work and/or cell phone number(s), and pharmacy you use for your prescription needs.
Be open and honest with us about your health history and health care needs (including your past and current illnesses, allergies, medications and dosages, and family health history).
Please provide us with at least 24 hours’ notice if you are unable to attend your appointment so that another patient can be booked into that time slot. Missed appointments will result in a service charge.
Treat (administrative) staff and other patients and visitors with consideration and respect. Understand that we will not tolerate violence, abuse (verbal or physical) or aggressive behaviour.
Work with the clinicians to build a relationship that is based on trust, honesty, respect and a mutual desire to improve your health outcomes.
