The Listowel- Wingham and Area Family Health Team is comprised of all physicians in Listowel and has been in operation for several years. Much of the success of the Listowel-Wingham and Area Family Health Team is due to the close working relationships with the Family Health Network. Their support of the collaborative practice model has contributed significantly in creating a framework to guide the future delivery of primary health care services by the Listowel-Wingham & Area Family Health Team.
Registered Nurses, Registered Practical Nurses and Nurse Practioners are key members of the Clinic/FHT as they develop programs and services tailored to the needs of the community. Either directly or together with the Interdisciplinary Health Care Professionals and other community service providers, they are instrumental in delivering a broad range of integrated programs targeted at the prevention and early identification of disease.
Nurse practitioners can provide primary care to all members of the Listowel Clinic. Nurse Practitioners are Registered Nurses who have additional formal education to advance their skills. Nurse Practitioners can assess, diagnose, prescribe medications, order laboratory and diagnositic tests.
The administrative staff of the Clinic/LWAFHT provides support to the Interdisciplinary Health Care Professionals in order to assist in the delivery of programs and services to the residents of North Perth. They help to administer quality services that meet the needs of our community in an efficient and highly effective manner.
The allied support staff of the Clinic/LWAFHT provides support to the Interdisciplinary Health Care Professionals.
